Individual Membership in only for Members of an Affiliate Under the RAJASTHANI HEALTH FOUNDATION.

Benefits

As part of our service to the community, the Foundation will provide the best possible medical help to all its members. Its Affiliated Association members and their families. Persons desirous of availing of these facilities have to register themselves with the Rajasthani Health Foundation.

Health Card

We will issue ad ID Card called HEALTH CARD to the applicants. It is a SMART CARD with all details like Names, address and colour photograph of the holder with Registration Number duty authenticated by the Chairman of the Foundation. The card should be signed by the holder on the reverse side immediately on issue and kept in safe custody. The card is valid for a period of one year, five year, and ten years (from the date of issue and not trasferable) depending upon the subscription selected. The Application should be made through the respective Affiliated Association ONLY.
